VEXUS FIBER VOICE E911 SERVICES ACKNOWLEDGEMENT

This VoIP 911 Service Disclaimer and Acknowledgement constitute a part of your Agreement with Vexus Fiber. By acknowledging this 911 Disclaimer, You and anyone using the VoIP 911 Services provided to you by Vexus Fiber understand and acknowledge the limitations of VoIP 911 services.

Before You begin using Vexus Fiber's VoIP Service(s), there are a few important things to understand about the Service(s) and its 911 and E911 emergency calling limitations as compared to traditional 911 and E911 service. Vexus Fiber's VoIP 911 and E911 Services may be limited or unavailable in the following circumstances:

• If you register an incorrect service address.
• If your telephone number and physical location do not correspond with each other.
• If there are delays in making your Registered Location available to the database accessed by the 911 Operator.
• If you fail to notify Vexus Fiber of a change in your physical location.
• If there is an outage, degradation or other disruption of electric power at your location.
• If there is an outage, congestion, degradation or other disruption of your broadband Internet connection.
• If there are delays or disruption of Service in the network of Vexus Fiber's underlying E911 provider.
• If your VOIP Service is interrupted or terminated for any reasons, including suspension or termination of your account for nonpayment or improper use of the Service.

By selecting "I Acknowledge" below, You understand the limitations of Vexus Fiber's VoIP 911 and E911 Services and agree to abide by Vexus Fiber's terms and conditions. You also acknowledge that You are fully responsible at all times during the term of the Services for notifying anyone that uses the Services of the 911 AND E911 limitations described to You above.
